Community Health Needs Assessment

A Community Health Needs Assessment (CHNA) is conducted to provide an understanding of the state of health in a community and the social factors contributing to and influencing health in the area. The CHNA may then be used as a guide for development of community health improvement strategies.

The periodic updating of assessments reflects changes in health status and factors over time and help ensure ongoing improvement efforts are based on the current needs of the community.

Saint Thomas Health and its member hospitals, conduct Community Health Needs Assessments (CHNA) of the communities it serves every three years. These assessments of unmet health needs provide a basis for addressing the health needs of the communities we serve, and a reference for each of the hospital’s implementation strategy (community benefit plan) ensuring our work within our communities are aligned with the community needs and our Saint Thomas Health mission.

We are looking for opportunities to join with others on initiatives addressing these health needs, as well as to foster collaboration on new initiatives. We invite you to present project proposals that will further community health in each of the areas we serve.

Whatever affects one directly, affects all indirectly. I can never be what I ought to be until you are what you ought to be. This is the interrelated structure of reality.
